IMPORTANT CHANGES TO OUR MEDICAL PLAN EFFECTIVE MAY 12, 2023
Our country has been under a Public Health Emergency since
January 31, 2020. During this time, our Medical Plan has offered unique and
temporary benefits to help members cover costs related to COVID-19. The Public
Health Emergency will end on May 11, 2023 which means the temporary benefits
will be terminated effective May 12, 2023.
What does this mean if I am enrolled in City of Detroit
Lakes medical plan?
COVID-19 Test and Antibody Test received at a clinic

Effective May 12, 2023, expenses for
testing received at a clinic will be subject to plan benefits. This means you may
be responsible for the cost of the office visit, COVID-19 test and Antibody
test when received. Remember that the overall cost of your visit and test will
be less when administered at an in-network provider.
COVID-19 Test purchased over the counter

Effective May 12, 2023, you will
be responsible for the cost of any over the counter COVID-19 test
purchased. The cost of the test will not be reimbursable under the medical
plan. You can, however, use your Health Saving Account (H.S.A.) dollars to pay
for the purchased over the counter COVID-19 test.
COVID-19 Vaccinations and Boosters

COVID-19 vaccinations and boosters
will remain covered at 100% under Preventive Care when received at an
in-network clinic. Any vaccination or booster received at an out-of-network
clinic will be subject to the out-of-network Preventive Care benefit level
highlighted on the SBC.
