Welcome to Open Enrollment!

Open Enrollment is the one time of year when you can make changes to your Employee Benefits enrollment.  Changes made now will go into effect August 1st, 2024.

Enrollment Instructions

• MANDATORY:  Please click the link below to view a short video that discusses your 2024-2025 benefit options, and Open Enrollment instructions. Please watch this video in its entirety.

• MANDATORY: Every Benefits eligible employee, whether enrolling or waiving, must complete the "Benefits Election form" located under the "Forms" tab above.
• If you are making changes to your current enrollment such as, changing your health plan or adding or dropping coverage for yourself or your dependents on any available benefit, you must complete the applicable form located under the “Forms” tab above.
• If you are not currently enrolled in the Optum HSA, and are enrolled in one of the HDHP options, please complete the "Optum HSA Authorization form" located under the "Forms" tab above if you would like to open an HSA. 

Please return all forms to your Business Office Manager, Administrator/Executive Director or email to HR@lantisnet.com as soon as they are completed, but no later than July 24th, 2024.
